I have an idea. What about having a model eat a teaspoon of cinnamon, breathe in fine ground black pepper through the nose, or similar, while really desperate? Coughing or sneezing will probably make it really hard to hold on. Maybe use this idea together with other ideas in a game setting?
Can anyone think of other ways of making people cough or sneeze?

I think this is straying rather far from the premise of desperation to pee and accidental wetting.
